چکیده

Heparin-induced thrombocytopenia (HIT) type II is a highly morbid and potentially life-threatening condition with limited treatment options in older patients at high risk of bleeding who develop acute kidney injury (AKI). The recent study by Tardy-Poncet et al. [1] showing that argatroban may be a safe and valid therapeutic option in this patient population is therefore of utmost clinical importance. However, when discussing other alternative therapies for HIT type II, the authors did not mention recent experience with fondaparinux, a selective synthetic antithrombin-mediated inhibitor of coagulation factor Xa [2]. Fondaparinux has often been used off-label to treat HIT type II in patients without AKI. Maximal treatment efficacy was obtained in all patients at an approximately sixfold less bleeding risk than in subjects treated with direct thrombin inhibitors (DTIs), including argatroban [3]. As

Anticoagulation in a Neurosurgical Patient with Heparin-Induced Thrombocytopenia Type II with Argatroban and Fondaparinux after Clipping of an Intracranial Aneurysm.

BACKGROUND: Therapeutic anticoagulation in patients after a major neurosurgical procedure remains critical because of the risk of a major intracranial bleed. Novel drugs could play a beneficial role in this setting. CASE REPORT: We describe a patient with heparin-induced thrombocytopenia type II and pulmonary embolism who was anticoagulated with argatroban and, later, with fondaparinux. No intr...

Fondaparinux for thromboembolic treatment and prophylaxis of heparin-induced thrombocytopenia.

OBJECTIVE To review literature evaluating the use of fondaparinux for thromboembolic treatment and prophylaxis in patients with heparin-induced thrombocytopenia (HIT). DATA SOURCES A MEDLINE search (1966-February 2006) was conducted using the search terms fondaparinux, heparin, low-molecular-weight heparin, and thrombocytopenia to identify English-language articles. Additional sources were id...

Fondaparinux-associated heparin-induced thrombocytopenia.

Heparin-induced thrombocytopenia (HIT) is an immune-mediated clinical syndrome of thrombocytopenia and thrombosis occurring after exposure to heparin. HIT most commonly occurs after exposure to unfractionated heparin but has also been reported with low-molecular-weight heparin. To date, there have been very few reports of HIT with the pentasaccharide fondaparinux, and some have proposed fondapa...

Heparin-induced thrombocytopenia: pathophysiology and new treatment options.

Heparin induced thrombocytopenia (HIT) is a severe complication of heparin therapy. It is generally accompanied by a paradoxical decrease in platelets leading to activation of platelets and of the coagulation system. HIT type I is a mild, transient, non-immune disorder. HIT type II is an immune-mediated reaction towards neo-antigen on PF4, which is platelet factor 4 (PF4) that is exposed upon b...

Successful management of acute thromboembolic disease complicated with heparin induced thrombocytopenia type II (HIT II): a case series

Heparin-induced thrombocytopenia type II (HIT II) is a rare immune-mediated complication of heparin. The diagnosis of HIT is considered in patients exposed to heparin, presenting with thrombocytopenia and thrombosis. We present two cases with massive pulmonary embolism and HIT, successfully treated with the administration of fondaparinux, an alternative anticoagulant, combined with the insertion...
